i just got my NSO certificate of live birth today! and i was hopeless when i found out that there was a typographical error between my middle and family name. instead of Arvin Morales Sanchez, it was changed to Arvin Bonjoc Morales which happened to be the maiden name of my mother. at the time that i was born, my parents didn't receive the sacrament of matrimony so i was suspecting that this is basically the reason! if ever i won't deal with this problem, could i possibly use the exact name stated in the NSO record and disregard my used name? hope you will give me a remedy to this chaotic situation. I guess you could not disregard the name you are using because legality wise, you will be obliged whether you like it or not to present ID including presenting requirements for application of passport, school matters...

I guess it would be more practical for you to undergo the process of correction of only 1 document (which is your NSO birth certificate) than to change everything you have from the day you started using your real name in all your legal and paper transactions. actually i visited our school registrar last Saturday March 20, 2010 and shared my problem. He advices me to file a correction of name at our school registrar since I am a graduating student and I really need an ample of time to correct those errors! Because basically you need to hire a lawyer and attend certain hearings which I can't possibly attend to because I'm still having my classes this summer. So the said remedy would be a great option and later on if I'll get hired with a job that would be the best time to file or to correct those errors.

Would be a filing of correction of name in our school registrar better than to have it change right away considering that I need much time to deal with it?

Hi, I need an advice or rather help on my spouce NSO Birth Certificate. The date of birth was incorrect. It should be March 17, 1972 but instead it was January 17, 1975. But in all his other documents like baptismal and school records it was March 17, 1972. His father who registered him was illiterate, or can't read and write, the only words he could wrote was his name. So, we don't know if the mistake was with the one who took the information or with his father.

He never requested BC before because baptismal was accepted in school and even when we got married in 1990. It was just recently we found it out when he need to apply for a passport and needs NSO BC.

Please help us. Thank you in advance for any help and assistance that you'll render us regarding this matter.

Hello poh & good day!...ang aking fullname na ginagamit since birth at lahat ng aking documents ay rowena canuel empig ngayun poh nung kumuha ako ng birth cert ko sa nso naging rowena canuel cañada na apelyido ng tatay ko sa nanay nya at ang pirma nya ay pedro cañada din hindi po nya nilagay ang ang apelyido sa tatay nya at nung time daw na yun ay hind pa kasal ang aking magulang...bakit poh nung kumuha ako ng birth cert ko sa nso year 2000 ang sabi po sa nso ay no records daw poh ako kaya ang nanay ko pinapunta ko sa quezon city para magrequest sa local registrar ang sabi din po no records din daw po ako ang inadvice po sa kanya dun ipalate registration na lang po ako kaya ganun po ang ginawa nya, na bigyan po ako ng late registration na rowena canuel empig at un na po ang ginamit ko gang sa mga ids at lahat ng aking documents..kumuha po kc ako ng passport ngayon nagkakaproblema ako sa aking nso birth cert ang aking mga ids at marriage contract ay rowena canuel empig..ano po ba ang dapat kong gawin? sna po ay madvice mo poh ako maraming salamat poh...

Hi, just want to seek an advice from you. My first name is spelled as JENNIFER with double "N" in all my records (school, employment & other gov't records). Last 2003 i had a chance to secure a passport. However, this is issued with my name spelled as JENIFFER with double "F" followed with my name in my NSO BC.
Question: How can i correct the entry in my BC? since i want it to be in JENNIFER, spelled with double "N"? I want my renewal of passport appeared in this name also.

I don't know if she registered this name but this new name I have used through the years that followed. Therefore, all of my records, such as school records, bear the new name. Hi! I have a problem with my marriage certificate. My marriage certificate is missing my 2nd name and I would like to have it corrected...How I can I do this??? Is there anyone in NSO that I can contact though... I had a lawyer who supposedly "fixed" it. And I got my 1st copy of my NSO Certified Marriage Certificate w/an annotation that my name was changed... But then when I got another copy of my NSO certificate months after, there was NO CHANGE in it. Not even an annotation.Was I being faked? Please help. Hi I need help with my marriage certificate. It lacks my second name. I hired a "lawyer" before and got me my marriage cert w/ annotation that says my name has been changed. Months after, I called NSO for another copy but then there was no annotation anymore. So I think I was faked by this lawyer. Problem is, I used my first copy of my NSO certified marriage cert to get my passport, now, what if I want to renew my passport, will there be legal repercussions on my part? Please help. Hi, I also need an advice regarding this. I do have a mistake in my Birth Certificate, instead of December 31 1989, it becomes December 31, 1990 in their records.

What makes me even more confused is that, the details below(e.g Certificate Of Attendant At Birth, Informant, Prepared By sections etc) has 1989 on it..

So the only mistyped detail is the "Date Of Birth Section right under my name.

I don't know how this mistake will affect my documents, can you please enlighten me? Especially, this certificate is a requirement for my work... Won't my papers be processed because of this? Will I be put on hold? My hiring?

Hello. I am Julius, of legal age and was born on Brgy. Malapit, San Isidro, Nueva Ecija.
My concern is about my Birth Cert from NSO, much to my surprise the "place of registration" registered in your good Office is a hand-written "Cabanatuan City" , but behind it there is a slightly visible type-written "San Isidro", different from the Cert. of Live Birth secured from the LCR of San Isidro, N.E. the place of registration (Legal and Correct) is SAN ISIDRO, NUEVA ECIJA only. Our L.C.R. were unfamiliar with this kind of case, the Chief of LCR told me yesterday that he transmitted my concerns as well as the supporting documents at N.S.O.- Legal, Manila. I asked him for the transmittal or LBC receipt or any document that can be used to file it on your office, but I receive nothing from him (LCR). So So i had doubt that he did not take actions, I can't blame them because my case is rare. I am familiar with RA 9048 but confused if my case is covered by these Act. So I would like to seek for a legal help on how to correct my B.C.. It is my desire to be registered "SAN ISIDRO" (legal and correct) as the "place of registration".

Hi. Got a problem. My Birth Cert from the Local Registrar is July 15,1969 gender is Female. I have been using these info eversince until I got a copy of NSO BC. I was surprised to see that my bday becomes July 5 and gender is now Male.

What are the things/docs to prepare so that an NSO lawyer can work on the change?

Good afternoon. I would like to seek an advice. My husband requested for a copy of his NSO Birth certificate. We did not expect any because he has been an OFW before and he was just issued from their local civil registrar in Cagayan that his records were among those eaten by white "anay". Now that our immigrant petition is in process, he needs to obtain an NSO negative birth certificate as a requirement among others, to get a passport renewal with the DFA. We were surprised that the birth certificate delivered to us was a wrong person, his older sister. When we complained, we were told that NSO cannot issue a negative certificate for my husband because my sister's birth certificate used the same birth year of my husband which was 1952. (The sister's real birth year was 1951.) The sister was willing to accompany us to NSO to attest to the fact that there was an error in her birth year. When we went to NSO, we were told by the Window 20 personnel that there should be a cancellation of the sister's birth certificate entry thru court before a negative cert can be issued to my husband. The sister is no longer willing to cooperate if she still has to go thru court. Is there any other way to do this? Court cases could be long and my husband and I are already 58 and 57 yrs old. We have lost our job, our properties and we don't have "kabuhayan" in the Philippines. We thought that now that US is already processing our petition, this is a God sent opportunity for us to rebuild our lives. What is the easiest remedy to our problem? How long will this kind of remedy be expected to take place? How much will it cost us? Pls. respond to me the soonest possible. We're almost desperate. I have problem with my Birth record.
I don’t have middle name in both of my Local copy and NSO birth certificate.
Also, my mothers’ last name is spelled incorrectly on the last letter.

I am in a hurry to get a PASSPORT. Can I just request a Joint Affidavit and have it presented temporarily at the DFA to get my passport while I am still on the process of correcting my wrong entries of my b.c?

NSO Lawyer,how can i correct this biggest problem of mine. Now ko lang po kasi natuklasan na ang name ko sa School ko ay hindi magkatulad sa Birth Certificate ko.

Good day, I am seeking for an advice regarding with my birth certificate. 1st year high school po ako at dito po ako nag aaral sa Cebu, but I am really from the province of Negros Occidental. By the time po na lumabas ang ni request ko po na bc I so hopeless that it is written their that I dont have record in National Statistics office at don ko lang din po nalaman na kahit po pala sa local registrar ay wala din po akong record, so I asked my mother to register me and it was already a late registration which she was applied last august this year but my problem now is my teacher asking me to give my nso birth certificate to her as soon as possible and I dont know if it could be possible ba na makakuha na ako ng bc ko na kaka register ko lang nong last august nga.. and if ever na kukuha po ba ako uli sa nso meron napo ba kaya akong record by that time? and if still wala parin po ano po kaya ang mga dapat kung gawin at ano din po ba ang mga requirements na kailangan ko pong dalhin para makakuha napo ako ng nso birth certificate... reply po asap...

my problem is the date of my birth, i used 9/5/64 since birth and to all my important documents, but when i get married we couldn't find my birth certificate and according to local registrar it was destroyed, so we did late registration, (using my baptismal cert dated october 30/64). anyway, we can managed to sorted out everything step by step,i had a copy of bc dated 9/5/64, no problem at all, until 2 years ago when i needed another copy of bc, it showed that my birthdate is October 5. (10/5/64) how come that my birthdate is oct.5, and i was baptized 6 days before i was born??? i want to change it but the nso office said, it's a long processing coz i'm using a multiple birthdate? but whose fault is this? i wonder if you can tell me what to do and if...how much does it cost to me this problem? thank you and hope to hear from your reply. god bless...

Hello! My daughter's BC has a major error on it. For some reason the nurse put down my nationality as "natural born Filipino", probably because I was born here. However, it should have been noted as "American" as I don't have Philippine citizenship. I don't know how I missed this (probably because I had been in labor for 29 hours?) but now they won't allow me to get a passport for my daughter and we're supposed to be leaving the Philippines next year. my problem is that i used November 9 1977 since birth and to all my important documents . I went abroad 2001 using the color green BC from the local civil registrar I got my first passport. When my passport expires I was required to submit the BC from NSO I was surprised having two records (two birthdates Nov 9 1977 & Nov 24 1976) Since I need to go back to my work ASAP the DFA told me to submit affidavit of explaination and to correct my all records (school, comelec) and I did. I was issued a new passport using the birthdate Nov 24 1976. When I apply for a visa it was denied for the reason I have two birthdates they want to see the annotations in my BC that there is a change in my birthdate. Do I need to file to get a court order or need to go the NSO head office to get an annotations. Need your help.

I want to merry my philippine girlfriend and she has only ONE LETTER written wrong in her MIDDLE NAME on her Birth Certificate (Original and NSO). Do you think its needed to change it,or can we get a Visa also with this clirical error, when every other documents is also written like this. I have a problem regarding my sister's gender.. it's so happen that her gender was crossed x in the male box portion appeared on the certificate of birth. By that, my mother asked the Local Civil Registrar (LCR)Attendant about this matter, according to my mother, replacing for the gender of sister needs court hearing and you'll be paying for 30,000 Pesos because it will be published on the newspaper. Is this for real? Is there any other way for this case to be solved?

Need your help/advice regarding my Marriage Contract. I found out that the middle initial of my wife was wrong. Instead of "C" naging "D" as written at the contracting parties portion. Pero pag dating mo sa portion na sa "THIS IS TO CERTIFY: That I, Tama ang kanyanyang Middle initial. Correct din ang Middle initial sa CERTIFY: That on the date... portion. Good afternoon, I recently received the birth certificate of my new born daughter from the Taguig City hall and the gender and address are wrong. They could not make the corrections anymore as the document had been passed on to the NSO. What is the most pain-free route I can go through to have this corrected and what are the requirements. In the birth certificate of my son, there is a place and date of marriage occurred. During that time there is this new family code that illegitimate children cannot use their father's surname. So we decided to fill in the blanks so my son can use my surname. And after a year they change the family codes law are back to the old one. (What a government???) Up to now, we are still single. Now that her mother is petitioning him. The embassy is asking for annulment records. We cannot provide annulment records because there was no marriage happened. What are we going to do? my mom filed for my birth certificate when i was born in 1986 but they filed it w/ a diff father's name and place of brith. when my mom had her second husband w/c is my biological dad they filed again another birth certificate with a diff father's name, surname and place of birth which is angeles city where they filed the late registration. but everytime i will go to nso and apply for a birth certificate i always get the first one that was filed. nso people told me that the late registered BC will not come out unless the original will be deleted. but deletin it would cost us a lot of money. what can we do?

Hello NSO Staff, Pinanganak po ako na hindi na registered, so recently nagparegistered po ako as late registered child. Kaya lang ang City Hall staff nag advised sa akin na para ma registered kailangan maglagay nang Parent's date of marriage which they put May 20,1977. Pero actually last Nov. 2005 lang po sila nagpakasal. Kailangan ko po sila e-petition dito sa Amerika. Kaya lang hindi magkatugma ang date of marriage nila sa birth certificate ko. Please po. @venus..the marriage is still valid..that is only an irregularity which will not affect the validity of the marriage..

@ mary you need to file a petition in court who has jurisdiction over the local civil registrar where your birth certificate was registered..it will cost you around 30 k.

Have fiancee that has wrong spelling on birth certificate. I gave her the money to get fixed and her mom went to "lawyer". He brought back a fake one. We need to apply for a visa and passport for her to come to US on fiancee visa. Is there any NSO lawyers in Mainla or in United States that can help me?

NSO Blocked my mothers 1963 LATE REGISTRY because unfortunately an older 1961 Birth Certificate has been found. We have been using her LATE REGISTRY 1963 for almost 20 Years and why did only now these 1961 Birth Certificate popped out when all Documents have been following the 1963 LATE REGISTRY. If this have been found earlier these problems about documents not tallying would have been repaired.

We already filed a case in court and we were denied a grant to use her 1963 Late Registry.

My Grandmother cannot read or write and also suffers from Alzheimer disease and just recently passed away. My mother only believed on my Grandmothers and Grandfathers statement about her Birth year and when she went to NSO to look for a Birth Certificate 20 years ago there were no found 1961 Birth Certificate so she filled a Late Registry 1963 year of birth. When she went to NSO these year to get a Late Registry they denied us. They are now showing us a Birth certificate year of birth 1961 that has a supplemental report of corrected name from baby girl to my mothers name. My mother has an older sister that already passed away and her birth certificate was younger than my mother. My aunt has lots of birth certificate that doesn't correspond to my mothers birth year 1963...

We really need the 1963 Late Registry even just a copy. Our Japanese Visa is in great jeopardy if we change my mothers birth certificate here. What would they say. Will they understand our systems in NSO. Would they accept court orders to undo the filed 1963 late registry that it is not my mothers fault without consequences of denying our visa. If we follow the 1961 Birth certificate what would happen to all of our document already followed the 1963 Late registry. We have been using the Late registry for almost 20 years and NSO just decided to block it without hearing our side even the court. NSO has a fault too why only now this 1961 Birth certificate found. Did they not check their system when there are other Birth certificate before granting a Late registry. How do we file for an appeal? Is it possible to put another supplemental report on the 1961 Birth certificate that it was the 1963 Late registry. Could you help me obtain a Late Registry 1963 of my mother. We are extracting every possible options.

i also have the same problem with tricia, how come we suffer the negligence of a person in the past, its only typographical, as i seek advice from pao i need to file my case in court and approximately i have to spend at least 10k just to correct my gender from male to female which in fact i have a child, and i have to undergo ultrasound just to prove that i have uterus to prove that i am a female, i think its ridiculous, is birth certificate of my son not enough to prove that i am a legitimate female? is our congressmen pass any bill regarding this problem?

Hi my name is Russky, I have a problem with my mum's birth certificate. She was born on 1954 by a hilot. According to my mum she was named as Norma but when she started school for a reason that she could not recall she started using Belinda until now. Recently we are trying to secure her NSO live birth certificate but found NO RECORD. We also tried in City Civil Registrar but they have no record as well for either Norma or Belinda. We thought of applying for a late registration but when we did that a question from the registrar's office came up, they said that there is a problem since my mum was born on 1954 and my grand parents got married two years after that which is 1956. In short my mum should not be using my grandfather's surname. The registrar's office then ask for any document signed by my grandfather stating his relationship to my mum but the worse thing is my grandfather died 2003 and my grandmother died last 2009. We could not find any records that could support my mum's status. I desperately need your advise...pLease

Got married last Dec 11, 2011 but when i checked it they entered wrong date of birth instead of June 27, 1981 they typed June 26, 1981.. Will this affect the validity of our marriage contract? what can we do to correct it?

my brother have a problem on his birth certificate on the gender. NSO copy didnt indicate the gender but the mandaluyong city hall has indicated the gender on his birth certificate. now the mandaluyong civil registar say that they can not provide a supplementary report but only indorsement letter that their copy of birth certificate have a gender. but the nso still asking that mandaluyong city should issue a suplementary report. is their anyone can give me an advice or any legal lawyer from nso that i can contact regarding this matter. please email me at my email add. philip_martin_rendado@hotmail.com

Hello, I also have a problem with my NSO. Recently found out that I do not have a MIDDLE NAME on it when I tried to process an SSS LOAN application. SSS rep advised that I get an affidavit or something to prove my identity. I will soon process my passport too and hope to get it straighten out. What do I do to correct this once and for all?

PS: I am an illeg. child if that helps :)

Hi poh! Almost 5 months n pong pinaprocess ng sister q ung birth certificate ng friend q.. Last September p poh last year,, We're here in saudi arabia riyadh and my friend gave an authorization to my sister to process his birth certificate. The problem is, nung first n dpat ire2lease n nila ung birth certificate, maxiado daw pong blurred. so they asked my sister to get a certified copy s bohol because taga bohol po ung friend q,, but sad to say, nung dumating ung copy, nde p rin nila nirelease kaze daw kelangan uli ng bagong copy,, so nag asked n nman kame to get another copy s bohol. after 2 weeks, nde p rin nila nirelease kaze kelangan nman daw uli ng new copy.. dumaan ang xmas, new year and til now nde p rin nila nire2lease kze kelangan nman daw nila ng certified photocopy ng registry book.. sorry sir/ma'am, my point is sana nman po pinagsabay sabay nila kung anu tlga ang kelangan kze kawawa nman ung matanda s bohol n pabalik balik s municipal hall nila to process the documents,, 5 months n kame naghi2ntay but still nde nmen alam kung anu tlga ang requirements nila. Really and to be honest, I am so upset with the processing of the birth certificate.. Please help us,, The name of my friend is Mr. Uriel Mascarinas. And sorry for telling you all of this.

While looking at my husband's original birth certificate, I noticed that it did not list his name (first, middle, last) although all other details are there, including his parents' signatures. Furthermore, his mother's middle name is misspelled. He recently applied for an NSO birth certificate and he's wondering what details will be on it if his original is full of errors.

I have long been using October 5, 1955 as my date of birth based on a certification of fire loss by the local civil registrar and baptismal certificate for all my records such as school records, employment, and even marriage contract. Just recently I discovered from NSO that October 10, 1955 was reported by my father as my date of birth.

Please advise as to steps to take.

Hi, I am 28y/o and newly married. We just got our marriage certificate 2 days ago when I realized that my age on the marriage certificate was 27. I was 27 when I applied for the Marriage License which they probably used as basis for the Marriage Certificate, but then my actual age at the time of marriage was 28. The birth date indicated though is correct.

Please help advise if (1) it is still necessary to correct the age indicated in the marriage contract, even if basic subtraction would surface my true age at the time of marriage; (2) it could be considered clerical error and if it could be corrected just by presenting the necessary supporting documents to the city register; and (3) other steps I would have to follow/comply with, subject to your response in Q1, to correct the record.

The error on my birth cert. is the place of birth w/c is Pilar, Capiz instead of Balasan, Iloilo. My baptismal is Balasan, Iloilo for I was baptized at Balasan, Iloilo but later transferred residence at Pilar, Capiz and issued birth certificate there at Pilar, Capiz during that time. I used Balasan, Iloilo as my birthplace in my school records and SSS during my previous employment but I also tried using Pilar, Capiz as my birthplace in my PAG-IBIG Fund. Now, I need an authenticated Birth Certificate for the Board Exam. What will be the possible solution for my problem? Please help me po! Hoping for your immediate concern w/ my problem. Thank you!

got really a big problem on my birth certificate and baptismal.my middle name and surname in my birth cert.was misspelled,but on my baptismal it is correctly spelled but my first name naman ang mali..the marriage certificate and birth certificate of my parents shows no records at nso.ang hirap din kc yung bothers and sister ko mali din ang surname at middle name.we are 5 na magkakapatid at isa lng sa amin ang me tamang papeles,kung sino pa ang pinanganak sa hilot.what will i do,ang required to correct is at least 2 of brother or sister na me correct document.please hellp,hindi din makakuha ng passport brother ko dhil dito.pls. advse on what to do. How do we correct entries on marriage contract, birthdate of spouse should be may 11, 1971 and not april 11, 1971. All other documents like passport and seaman's book were already corrected except sss ID and marriage contract. My husband was still on the process of correcting his date of birth when we got married, thus the wrong details on the marriage cert. Documents submitted to the church bore the wrong dates.

They found out that his last name spelling was different from his birth certificate to his marriage certificate. My question, is his first marriage valid with that not the same spelling?
